外籍人士來台短期就醫申請停留簽證手續說明 Procedures of short-term stay visa application for foreigners coming to Taiwan for medical treatment
應繳文件 Required Documents	說明Instruction
簽證申請表 Visa application form	須至「線上填寫申請表專區」填寫，列印產出具有條碼之簽證申請表，並親自簽名確認。 Applicants are required to fill out the visa application form online in the designated section(https://visawebapp.boca.gov.tw/BOCA_EVISA/), print out the barcode-embedded visa application form, and sign it in person for confirmation.
6個月內2吋彩色照片2張 Two 2-inch color photos taken within the past 6 months.	背景須以白色為底色 The background should be white in color.
護照正本及影本 Original and photocopy of passport.	效期6個月以上且須有空白頁。 The passport must be valid for at least 6 months and have blank pages.
證明文件 Supporting documents	1、財力證明(例如銀行存款證明或與我國醫療機構達成之付款協議） Proof of financial capability (such as bank deposit certificate or payment agreement with medical institutions in Taiwan). 2、國外醫院診斷證明。 The diagnosis certificate from foreign hospitals 3、國內醫療機構療程安排及說明書。T reatment schedule and instruction letter from Taiwan medical institutions.
來回機票 Round-trip air ticket	機票、電子機票或旅行社證明 Flight ticket, e-ticket or certificate from travel agency
申請程序 Application process	由申請人檢具各項應備文件及簽證規費向中華民國駐外館處申請停留簽證入境。必要時中華民國駐外館處將要求申請人面談。 The applicant must prepare all required documents and the visa application fee, then submit the application to the ROC embassy/consulate abroad for a stay visa to enter Taiwan. When necessary, the ROC embassy/consulate abroad may request an interview with the applicant.
注意事項 Important notes	1	簽證核發為國家主權行為，中華民國政府有權拒發且無須說明原因。提出簽證申請者不論是否獲發簽證，已繳之簽證規費依法不退還。 The issuance of visas is an act of national sovereignty, and the government of the Republic of China has the right to refuse without explanation. Whether or not a visa is granted, the visa application fee is non-refundable by law.
	2	停留簽證規費及美國籍人士申請簽證之相對處理費收費數額，請參考「外國護照簽證收費數額表(doc檔案)」。 For the fees of stay visa and relative processing fees for U.S. citizens, please refer to the “Standard Fees for Republic of China (Taiwan) Visas in Foreign Passports(DOC)"
	3	得以來臺接受醫療服務為由申請簽證之疾病種類，比照衛生福利部公告之「外籍人士及大陸地區人民得於臺灣地區接受醫療服務之疾病」一覽表。 3. The types of diseases for applying for a visa on the grounds of receiving medical services in Taiwan shall refer to the “Medical Treatment for Curing Diseases Categorized in the 28 Disease Item List for Foreign Nationals and People from Mainland Area (PDF) as announced by the Ministry of Health and Welfare.
	4	國內醫療機構限衛生福利部公告之「醫療服務國際化推動計畫會員機構」。 4. Taiwan medical institutions are limited to members of the "Healthcare Service Internationalization Transformation Promotion Program (PDF) " announced by the Ministry of Health and Welfare.
	5	陪醫者以病患配偶或三親等內親屬2人同行為原則，必要時得增列居住國醫事人員2人隨行照顧。申請人得檢具親屬關係證明或醫事人員服務單位派遣證明，以陪同就醫為由申請來台簽證。 As a general rule, a maximum of two family members within three degrees of kinship to the patient's spouse may accompany the patient. If necessary, two medical personnel from the country of residence may be allowed to accompany the patient to provide medical care. Applicants may provide proof of kinship or a certification of assignment from their medical service unit to apply for a visa to accompany the patient for medical treatment in Taiwan.
	6	重大緊急醫療簽證申請案可由我國醫療機構向外交部領事事務局提出申請。 Emergency Medical Visa applications can be submitted by Taiwan medical institutions to the Bureau of Consular Affairs, Ministry of Foreign Affairs.
